The Office of Registration and Records is happy to provide the following document services for our current students and alums.
Due to the Family Educational Rights and Privacy Act, document requests cannot be emailed unless an electronic signature is used. All other requests must be mailed, faxed or personally submitted. For information regarding the laws governing the disclosure of student records, please see the Feinberg School of Medicine Access to Student Records Policy in the student handbook.
These services are for MD degrees only. This office may not process alumni document requests for dental, nursing, physician’s assistant, prosthetic-orthotics or physical therapy degrees. This office does not process alumni documents for certificate or PhD programs. Please go to the individual program websites, available via the Feinberg Education page, for assistance.

The Medical Student Performance Evaluation (MSPE) letter – formerly called the Dean’s Letter- accompanies a medical student’s residency application. It summarizes the student’s medical school performance and provides a summary evaluation of the student’s potential as a resident.
An MSPE may not be released directly to the MD student or alum. It must be sent directly to a third party. MSPSEs may be sent to programs for no additional charge, requested either via email to FSM_records@northwestern.edu, or as a supplemental document in Parchment with the official MD transcript.
A signed and sealed copy of an MD diploma (class of 1988 on) may be requested through Parchment. There are no additional fees when requested as a supplemental document with the official MD transcript.
The Feinberg Registration and Records team is happy to sign and seal a copy of a MD diploma pre-1988. Since we do not have copies on file, a PDF of the diploma must be forwarded by the alum to the Registration and Records team.
