Ordering

There is a multi-step process to submit and order or a service request to the MHPL. Please follow the below instructions accordingly. If you are ready to proceed with the MHPL Requisition Form, please follow the directions below.

  1. Access the NUCORE website
  2. Log into the NUCORE system (NU Network ID and password is the same for NUCORE). Please contact NUCORE Support if you do not have a NUCORE account or if you are unable to log in.
  3. On the HOME tab, scroll down and select the Mouse Histology and Phenotyping Laboratory (MHPL) link.
  4. Scroll to the Services section where it lists HISTOLOGY SERVICES. Please enter 1 (or more depending on your order) in the field next to “Histology Services” and click the CREATE ORDER button.
  5. You will be prompted to SELECT PAYMENT SOURCE for your order. Please select your payment source, and then click CONTINUE. If you do not have a payment source, please contact NUCORE Support
  6. Select from the CART screen COMPLETE ONLINE ORDER FORM
  7. You will be prompted to fill in the MHPL service request online form, which is pre-populated with your username, email address, department, payment source, NUCORE order number and the current date.
  8. Please fill in all necessary information and special instructions on this MHPL service request form to help MHPL process your order correctly.
  9. Please fill in the cells prompting for unique SAMPLE ID, type or organs/tissues and additional comments to provide information about each of your samples. If there are more than 25 specimens, please feel free to attach your own document/spreadsheet with the full specimen list or special instructions file for tissue embedding at the bottom of the online MHPL service request form
  10. NOTE: Please limit SAMPLE ID to 12 characters only since this as many characters that could be printed on microscope slides and tissue cassettes.
  11. Press DOWNLOAD PDF to obtain a printable version of the filled MHPL requisition form. Please review that the PDF shows all relevant information clearly (e.g., number of sections/slides, special stains, list of antibodies). Print two copies of this PDF and retain a copy for your own record. It is the responsibility of the users/researchers to keep a record of their requisition forms and sample lists.
  12. Press ATTACH TO CART.
  13. Do not forget to press PURCHASE in the next window to submit and complete your order. Orders that are not submitted to NUCORE will not be processed by MHPL. Failure to press PURCHASE invalidates the NUCORE ORDER number automatically generated by the requisition form software. You should get an automated e-mail from with the heading “NUCORE Order Notification”, once your order has been received.
  14. Submit your specimens to the core together with the completed requisition form and full SAMPLE LIST at a drop-off table outside Olson 8-333. MHPL staff will not accept samples nor initiate an order unless an NUCORE Order Notification has been generated.

Special Instructions for External Orders (including Lurie Children’s Hospital Clients)

Please create a purchase order (PO) and provide us a copy it. If you plan on placing multiple orders with our Facility, we recommend that a blanket/open purchase order be created. A blanket PO will allow you to continuously place orders against the same PO eliminating the need to create a PO every time that you place an order. Please have the payments sent to the following address:

Robert H. Lurie Comprehensive Cancer Center Northwestern University
676 N. St. Clair
Suite 1200
Chicago, IL 60611
Attn: Renay Wilson-Brown.

When submitting a new PO, email Brandon Stuart Baker to request that the PO be added to the NUCORE system. Orders cannot be placed using the NUCORE system if the PO is not set-up in the system.

Also provide the following information in the request:

  • Investigator name, email address and phone number
  • Technician name, email address and phone number
  • (Optional) Financial Administrator name, email address and phone number
  • The Institution’s Accounts Payable Department mailing address, email address and phone number

Please contact NUCORE Support if you need assistance using the NUCORE system or encounter any issues when placing an order.
