Most patients will have significant traumatic mechanisms, but the elderly or osteopenic patient may suffer spine fractures from simple falls.  Alert patients with C2 fractures will complain of neck pain and have tenderness to palpation on exam.  A neurologic exam should be performed, however, a normal neurologic exam does not rule out a fracture.
