A 62-year old female presents with 36 hours of acute pain over the left 1st MTP joint with swelling and erythema.  There has been no trauma, wounds, bites, or fevers.  She had the same thing last year, and it resolved in a couple days.  She takes HCTZ for hypertension.  What is the diagnosis?
