Osteomyelitis

47 year old male with no history of trauma and a past medical history of HIV who heard crunching in his neck with pain.  There is loss of vertebral body height of C3 and C4 with anterior wedging of C3 and retropulsion into the  spinal canal.  The prevertebral soft tissues are slightly widened.  The C3-4 intervertebral disc space is completely  destroyed.  These findings are suggestive of diskitis and osteomyelitis.

This is a diabetic who presented with foot pain. The plain film demonstrates 3rd metatarsal erosion and subcutaneous emphysema consistent with osteomyelitis.
