ED management includes orthopedic consultation to arrange for surgical repair by open reduction internal fixation with one or two syndesmotic screws between the tibia and fibula.  Some orthopedists may decide to assess the stability under anesthesia and manage the injury with a long leg cast with frequent follow-up.
