Maisonneuve Fracture

Diagnosis

Radiographs of the ankle may reveal no fracture or a medial malleolus avulsion.  There may be widening between the distal tibia and fibula.  The fibula fracture is usually proximal to a conventional ankle x ray, so if there is any proximal leg pain or tenderness to palpation, dedicated radiographs of the tibia-fibula should be ordered, which will readily diagnose the fracture.
