Counseling and Referral for Hypertensive Patients in the Emergency Department

Northwestern Memorial Hospital strives to provide the “Best Patient Experience.” In support of this mission, the Emergency Department (ED) has identified a patient population in need of educational intervention that would save lives and improve quality of life.

The ED has significant opportunity to participate in early detection and treatment by integrating routine screening and referral for patients identified with elevated blood pressure. This project seeks to develop an educational intervention by obtaining patient input, ultimately to be provided to patients at risk for undetected or uncontrolled HTN, and support “Best Patient Experience” mission by facilitating physician follow-up and referral.

The project has two goals: obtaining patient input in developing a discharge teaching intervention along with early detection and treatment of HTN in the ED; and enrolling 25 patients to evaluate the feasibility of a formal physician referral mechanism for high risk patients. Our long-term goal is enhancing the best patient experience through coordinating care to the unique needs of possible HTN patients by involving the patient in the development process.
