Cardiothoracic fellows will perform preoperative evaluations and in conjunction with residents, assist in writing preoperative orders and notes in the patient’s chart. The fellow will be intimately involved in the anesthetic selection and perioperative management of cardiovascular surgical patients, and discussing the cases with residents and the attending. Postoperatively, fellows will follow their cases in the intensive care unit and write postoperative notes on their patients’ charts. During their cardiovascular ICU rotation, fellows will round on all CV ICU patients, working closely with the residents, anesthesia CVICU attending, and surgeon. They will write daily notes on patients’ charts and write appropriate patient orders.
Cardiothoracic fellows will function as consultants with the cardiovascular ICU attending anesthesiologist, assisting in the postoperative management of CV ICU patients. The cardiothoracic fellows will work closely with the consulting cardiologists involved in intraoperative echocardiography during their 2 month echocardiography rotations, performing intraoperative real time evaluations and postoperative TEE reviews. Fellows will attend weekly cardiothoracic surgical, cardiology and anesthesiology CQI meetings.
Fellows will be taught from a multidisciplinary approach, including cardiology and CT surgery. They will also be involved with teaching residents and medical students on the clinical cardiac anesthesia rotations. Fellows will be involved in a didactic lecture series during the cardiac anesthesiology rotation for CA-2 and CA-3 residents. They will also be involved with hands on training of CA-2 and CA-3 residents in the O.R.
The Cardiothoracic Anesthesiology Fellowship Program is accredited by ACGME. Completion of the program meets requirements for the American Board of Anesthesiology special certification in Cardiothoracic Anesthesiology. The service is supervised by Anesthesiology attending physicians who are board-certified in Anesthesiology and certified in Perioperative Transesophageal Echocardiography.
Scholarly ActivitiesFellows will plan and conduct regular cardiac anesthesia lectures for attendings and cardiac anesthesia residents (6 one- half hour lectures each per year on advanced cardiac anesthesia topics). Each fellow will plan and conduct a one hour Grand Rounds presentation to the entire Anesthesia Department consisting of case presentations/review of literature/research topics in cardiac anesthesia. Fellows will plan and prepare, in conjunction with a faculty mentor, an academic project (case report, basic research) to submit as an abstract for the following year to the Society of Cardiovascular Anesthesiologists annual meeting.
Duty Hours & Call Responsibilities
Throughout their seven months of clinical cardiovascular anesthesia and when on call, fellows will be intimately involved in the preoperative, intraoperative, and postoperative management of cardiovascular, thoracic, and electrophysiologic surgical patients. The fellows will be responsible for daily clinical care beginning at 7am Monday through Friday. They will receive one non-clinical day weekly for academic and research pursuits. Home call will be assigned on a weekly basis. Fellows on call will be expected to report for surgeries when summoned by the attending on call. The 80 hour maximum in hospital work load policy as applied for residency guidelines will also be applied for fellows. Fellows working past midnight will not be responsible for working the next day. Appropriate food and lounge facilities will be provided. No call room will be required as fellows do not take in house overnight call.
