Neurosurgical Anesthesiology Fellowship Curriculum

The Section of Neurosurgical Anesthesia is devoted to the perioperative care of the neurosurgical patients. The Neurosurgical Anesthesia Fellowship training program is structured to teach safe, comprehensive and excellent anesthesia care. By the end of the fellowship, the fellow is expected to function as an independent consultant neurosurgical anesthesiologist.

Goals & Learning Objectives

Cranial Surgery

1-     Understand and manage patients with intracranial mass lesion

2-     Understand and mange patients with intracranial aneurysms

3-     Understand and mange patients with arterial venous malformations

4-     Manage and induce hypothermia

5-     Manage deliberate hypotension

6-     Manage anesthesia for neurophysiologic monitoring

7-     Be familiar and comfortable in neurophysiologic monitoring

8-     Be familiar and comfortable in brain protection during neurosurgery

9-     Manage intracranial hypertension

10-   Monitoring of the cranial nerves

11-   Monitoring patients in the sitting position and management of air embolism

12-   Anesthesia for carotid surgery (regional and general)

13-   Awake craniotomies

14-   Anesthesia for epilepsy surgery

15-   Anesthesia for deep brain stem stimulation

16-   Anesthesia for minimally invasive cranial surgery

17-   Anesthesia for skull base surgery

18-   Anesthesia pituitary and the hypothalamus

19-   Proficiency in invasive monitoring

Spine Procedures

1-     Management of spinal cord injuries

2-     Management of major spine procedures

3-     Massive transfusion of blood and blood products

4-     Coagulation problems in spine surgery

5-     Neurophysiologic monitoring in spinal surgery

6-     Complications in major spine in prone position and blindness

7-     Anesthesia for spinal cord stimulation

8-     Awake nasal fiber optic intubation

9-     Wake up test for major spine cases

Interventional Radiology (IR)

1-     Understanding and mastering anesthesia outside the operating room

2-     Manage anesthesia for ruptured aneurysm in the IR suite

3-     Manage anesthesia for arterial spasm in the IR suite

4-     Manage acute stroke patients in the IR

5-     Understanding and mastering the management of anesthesia for alcohol use in AVM patients

6-     Wada test

Neurosurgical Intensive Care

1-     Understand and mange post operative care in the NSICU

a.      Ventilation and arterial blood gasses management

b.      Cardiovascular stability

c.      Temperature control

d.      Nutrition

2-     Understand and manage ICP in the NSICU

3-     Post operative neurosurgical care

a.      Neurosurgical evaluations

b.      Understand the different neurosurgical tests

4-     Fluid management in the NSICU

5-     Brain protection

6-     Artificial pharmacological coma for brain protection

7-     Hypothermia in NSICU

8-     Managing spinal cord injured patients in the NSICU

a.      Airway management in cervical injuries

b.      Ventilation management in high spinal injuries

c.      Cardiovascular changes in high thoracic and cervical injuries

d.      Autonomic dysreflexia, diagnosis and treatment

e.      Hyperkalemia in spinal injured patients

Pediatric Neurosurgery (Children Memorial Hospital)

1-     Be familiar with neurosurgical anesthesia in pediatric patients

a.      Ventilation and arterial blood gasses management

b.      Cardiovascular stability

c.      Temperature control

d.     Nutrition

2-     Be comfortable with anesthesia in the pediatric group

3-     Fluid management in pediatric patients

4-     ICP management in pediatric patients

5-     Airway management in pediatric patients

6-     AVM in pediatric patients

7-     Spastic disease in pediatric patients under anesthesia

General Knowledge

Fellows will learn:

1.      Basic research methodology/statistical analysis

2.      Quality assurance and improvement

3.      Ethical and legal issues

4.      Practice management

Fellowship Scholarly Activity

1-     Fellows are expected to present three lectures- two at Neurosurgical Anesthesia Conferences and one at the Department Grand Rounds.

2-     Periodic journal review article presentation

3-     Preparation and publication of review article, book chapter, or manuals for teaching or clinical practice.

4-     Clinical or basic research investigation or similar activities.

Communicating the educational objectives to the fellows

The Program Director will review the learning objectives with the fellow before each rotation starts. Also, the fellow will have access to each rotation’s learning objectives in New Innovations.
