Coverage Selection Form InstructionsThe Coverage Selection Form is now available on-line. It can be accessed through your CAESAR Account. The 2010-11 annual premium cost of the Plan is $2,466. The cost to enroll a dependent spouse and child T.B.A. The 2010-11 academic year coverage begins September 1, 2010 and ends August 31, 2011.
For Incoming Medical Students and Medical Students Returning From A Personal Leave of Absence:
- All incoming medical students and medical students returning from a personal leave of absence must complete an on-line Coverage Selection Form (CSF) by October 1, 2010 to either enroll or waive participation in the University sponsored health insurance plan. Once a selection has been made, the selection will be ongoing, as long as their registration status at the University remains the same.
- All full-time students whose Coverage Selection Form is not completed online by October 1, 2010 will be automatically enrolled in the Student Health Insurance Plan, and will not be able to waive participation until the next open annual enrollment period next summer. For Continuing Medical Students:
- If you wish to retain your current 2009-10 status (enrolled or waived), there is nothing you need to do. Your prior academic year status will automatically roll over to the 2010-11 academic year.
- If you wish to change your insurance status for the 2010-11 academic year (enroll or waive), you must submit an on-line CSF form requesting such change. The CSF Should be submitted as soon as possible, but no later than October 1, 2010. Please scroll down to view on-line Coverage Selection Form completion.
- If you have a qualifying life event change (e.g. loss of parent's insurance coverage, marriage and/or birth) and you wish to enroll in the student insurance plan mid-year, download a copy of the Coverage Selection Form and submit by mail from the Student Insurance Website at: http://www-chicago.nuhs.northwestern.edu/ Be sure to include a copy of your insurance termination letter or a brief explanation for why you are requesting mid-year enrollment.
How To Complete The Coverage Selection Form On-Line: (On-line enrollment closes after October 1, 2010)
- Log on to your CAESER ACCOUNT with your NET I.D. and password at: http://www.northwestern.edu/caesar/ If you are a incoming student and do not have a NET I.D. and password, please contact the Feinberg Admissions Office at (312)503-8206
- From the Enterprise Menu, click on "View My Student ID, then click on "For Students, then "Enrollment", finally click on Health Coverage to submit the form.
- Once you have submitted your form on-line and you have been charged for the plan, you will not be able to make any changes on-line. You will need to contact the Student Insurance Office at (847)491-2113 or 2114 to make any changes.
