KYHH takes pride in the partnerships that it forms with community leaders and organizations around the Chicagoland area. As a group, we strive to establish lasting connections with health and service groups of all types who have a similar commitment to the wellness of their neighborhoods.
We partner with individuals and organizations hosting community events in order to provide heart health screening and counseling services. In the past, these have included health fairs, faith-based centers, neighborhood festivals and any other gathering that brings together the people of a community. Connections with these partners allow us meet Chicagoans where they live and to serve populations with high cardiovascular risk in culturally and community specific contexts.
We also work with nutrition resources, physical fitness centers, and Federally Qualified Health Centers in the Chicago neighborhoods in which we operate in order to provide reliable referral information and aid in access to health resources. With any group that we work with, our goal is to be able to provide counseling and recommendations that are specific and actionable for individuals we see and screen at our events. We believe that strong community connections are essential to creating change and improving health outcomes!
