How would you describe the importance of your work to a lay person with no background in population health?
Practice facilitation is becoming a new healthcare role to assist with quality improvement in patient communities as well as ambulatory care settings. It is useful to understand practice facilitation activities to determine if certain types of activities promote successful outcomes. Learning about these activities and their relationships can guide future practice facilitation in community settings.
