Position Appointment Forms

All appointment position forms must be signed by the department and routed to the Office of Finance and Administration or the Faculty Affairs Office (see workflow).

Appointment forms are used to alter information related to the employee who occupies the position, including termination.  Position forms are used to alter information related to the actual position.

Position appointment forms must be signed by the requesting department and sent to the appropriate department for Dean's Office signature - Finance and Administration, Faculty Affairs, or The Graduate School (TGS). 

The Personal Data Form is required for all new hires (including faculty, staff, and students) and is the primary form for hiring a temporary or work-study employee.  This form also is used for employee name and address change requests.
