Admission Procedures and Requirements
- Please start your application in November 2009 or later. All applications started prior to November will be automatically deleted from The Graduate School application system - 1. Northwestern University Graduate School online application: The Graduate School online application can be found on the Northwestern University web site at www.tgs.northwestern.edu/admission/. Please use Program Code H02ND. Please select Summer as the Admission Term. Once you access the online application, you will be given an application PIN and USERID. You may access the application as many times as necessary before submitting. When you are ready to submit, you will be prompted to pay the application fee of $75 which must be paid by credit card. If you have any difficulty accessing the online application, please call the technical support hotline at 1-800-526-3313 2. Letters of Recommendation: Applicants are required to submit two letters of recommendation which either can be submitted electronically via The Graduate School online application or by mail to: 3. Transcripts: Applicants are required to submit one set of original transcripts (bearing the registrar’s signature and/or institution’s seal) from each postsecondary institution attended. The transcripts should document all courses taken, grades received and (if applicable) degree earned. These should be mailed directly to: Graduate Programs in Healthcare Quality and Patient Safety 4. CV: Applicants are required to submit their most recent Curriculum Vitae which can either be sent via The Graduate School online application or via email to MastersIHS@northwestern.edu
