Members of the Northwestern Center for Patient Safety participated in the Chicago Patient Safety Forum's Annual Meeting. Northwestern contributed posters and presenters to the 2007 conference.

Department of Emergency Medicine and Center for Patient Safety faculty, Chris Beach, MD (Far left), and John Vozenilek, MD (far right) are joined by colleagues Abra Berg (mid left) and Dr. Sarah Donlan (mid right) with their poster titled "Transitioning Patients from the Ed to the Hospital: Observations of Handoff Communication." Leaders in HIT (Health Information Technology), Abel Kho, MD and David Liebovitz, MD present their poster during the 2007 Chicago Patient Safety Forum Annual Scientific Meeting. Their presentation was titled "Reduction in Preventable Codes by Implementation of a Rapid Response Team."
Kristine Gleason, RPh, and Molly McDaniel, PharmD, also from Northwestern Memorial Hospital, produced two posters based on their Medication Reconciliation and Medications at Transitions and Clinical Handoffs (MATCH) research.
