Improving Clinician Communication: Chart Review

AIM: To develop a medical chart review tool to measure problematic clinical communication

METHODS:

• Case-control study
• Cases = medical charts of cases with known medical error or communication risks
• Controls = matched by age, gender, admission period, and diagnosis
• Identify elements related to problematic communication
• Refine medical chart review tool
