- Conduct 60-90 minute, audio-taped focus groups at each hospital using a standardized protocol.
- 63 focus groups were conducted
- Inquiries about effective and problematic communication between clinicians, solutions, and “work arounds”
- Themes cover the breadth of clinician communication issues related to patient safety
- Results are aggregated across institutions
- Key findings included identifying several key contexts for communication: standardized communication, transitions of care, assessment of patient acuity, teamwork and team culture
