Purpose:

This form is used to request monthly account budget statements (FM160, FM162, and NG32) be added to an established Infopac.

NO new infopacs will be approved because current and historical budget statements are available online via View Direct.

Routing and Approval:

All Infopac Request Forms, with original signatures, must be sent to FSM-Finance & Budget for review and approval.
