All full-time regular and research faculty are required to file an annual Faculty Disclosure Form (Annual Disclosure form) with Medical School administration. This disclosure concerns activities which occurred during the previous calendar year, and the request to submit the form is generally sent to faculty in the spring each year. Disclosure of potential conflicts of commitment or conflicts of interest is not optional; those who fail to submit their disclosure forms on an annual basis place renewal of their faculty appointment in jeopardy when their current appointment expires.
