FAO PROMOTION PROCESS FORMS for 2008-09 (4)

  • Select the appropriate form for your promotion/tenure action
  • Use and complete one form for each promotion candidate
  • Submit the completed form with the other required materials

DEADLINES:   

September 22 for promotions reviewed by APT and/or AD HOC Committee

November 3 for all other promotions (to non-tenure-eligible Assistant Professor or Assistant Professor, Clinical, reviewed by Dean for Faculty Affairs)

HIGHLIGHTS OF THE 2008-09 PROMOTION/TENURE PROCESS:

1) CV GUIDELINES: CVs must be formatted according to the current guidelines or they’ll be returned to the Department for modification. DO NOT INCLUDE SSN ON THE CV.

2) SUBMITTING MATERIALS (please do not staple or clip any documents)

• FORM #1, CANDIDATES REVIEWED BY DEAN FOR FACULTY AFFAIRS: Submit 1 ORIGINAL PAPER FILE (no copies/no electronic files) in the order listed on Form #1. Mark your packet “Promotion + Department Name + Candidate Name”. NO PUBLICATIONS SHOULD BE SUBMITTED.

• FORM #2, CANDIDATES REVIEWED BY APT COMMITTEE: Submit 1 ORIGINAL PAPER FILE (no copies/no electronic files) in the order listed on Form #2. Mark your packet “Promotion + Department Name + Candidate Name”. NO PUBLICATIONS SHOULD BE SUBMITTED.

** NOTE: PROMOTIONS to PROFESSOR, CLINICAL: in addition to paper packet, please send an electronic CV to fao@northwestern.edu and submit a list of 8-10 suggested external referees; DO NOT REQUEST/SUBMIT LETTERS

• FORM # 3, CANDIDATES REVIEWED BY AD HOC COMMITTEE: All documents need to be submitted electronically as email attachments t fao@northwestern.edu. Also submit one paper copy to our office, including the completed Form #3.

• DO NOT SCAN CVs: Send CVs as a file attachment to fao@northwestern.edu.
• NAMING YOUR ELECTRONIC FILES: Please indicate Department + Last Name + Item
• SUGGESTED REFEREES: submit no more than 8-10 names
• DO NOT SUBMIT PUBLICATIONS

• FORM #4, RESEARCH FACULTY CANDIDATES REVIEWED BY APT COMMITTEE: Submit 1 ORIGINAL PAPER FILE (no copies/no electronic files) in the order listed on Form #4. Mark your packet “Promotion + Department Name + Candidate Name”.

3) CHANGES IN APPOINTMENT (tenure status or appointment basis) need to be submitted as separate requests PRIOR to promotion requests, effective no later than 9/1/08, before the promotion process can begin. If you’ve questions about the specific requirements for these changes or need confirmation of a current appointment, contact Solange, Vallea or Marcie.

4) CHANGES IN CAREER TRACK need to be agreed upon by the Department and faculty candidate and requested prior to 9/1/08. Exception: change from Undifferentiated to a specific track may be included w/the promotion request. Contact Marcie or Solange if you need to confirm the current career track.

5) EXTERNAL REFERENCES: The quality of these letters (prominence, rank, institution) is an important determinant of how smoothly the promotion will proceed. Poorly chosen external referees may adversely affect the likeliness of promotion success. Reference letters from the candidate’s training institution are discouraged. Please solicit letters from prominent people in the field with no training connection to your candidate, holding the same rank or higher than the proposed promotion.

• We recommend candidate submit a list of suggested referees to Dept. Departmental APT Committee and/or Dept Chair should review list & remove/add referees before soliciting letters.
• Identify rank and Institution for all referees on each submitted letter

**FACULTY DISCLOSURE (formerly known as CONFLICT OF INTEREST): Promotion candidates must have completed the 2007 form AND the 2008 form (available spring 2009) in order for the promotion process to proceed.
