M4 Clerkship Changes

• Use the M4 Drop/Add form .
• Print form, complete it and submit it to the designated departmental representative for approval. 
• Use the departmental contact list to identify the appropriate signatures to solicit.
• The departments will submit the signed form to the registrar.
