Feinberg, NMHC, Lurie Children's, and the Shirley Ryan AbilityLab are committed to conducting their business with integrity and transparency. As you know from your participation in the 2017 annual disclosure survey, a cornerstone of this commitment is the management of conflicts of interest through appropriate disclosure and the development of effective conflict management plans. Accordingly, you are once again required to participate in this process by completing an annual disclosure survey. The 2018 survey combines questions from Feinberg, NMHC, Lurie Children's, and the Shirley Ryan AbilityLab, thus, regardless of how many roles you may hold at one or more of these organizations, you are only expected to complete one survey.
The survey will be distributed to returning users via email on February 1, 2018. All users will be required to complete the annual disclosure survey on or before February 28, 2018.
