Fundamental to the missions of Northwestern University Feinberg School of Medicine (Feinberg), Northwestern Memorial HealthCare (NMHC), Northwestern Medical Group (NMG), Children’s Hospital of Chicago Medical Center (Lurie Children's), and the Rehabilitation Institute of Chicago (RIC) is the advancement of public health. This mission is critically dependent upon the public's trust in the integrity of the research, education, and patient care performed at our institutions. To that end, our academic medical community's relationships with industry or other external entities, which often result in breakthroughs in medical research and promising new therapies, should be encouraged, but also keenly monitored. These relationships must not be or appear to be influenced by factors other than the pursuit of knowledge and the best interests of the patient.

With this in mind, these Northwestern-affiliated entities have adopted policies that require physicians, faculty, management, and certain staff to disclose industry and certain other external relationships through an annual survey. Each person's responses are reviewed, and, if necessary, individual conflict management plans are developed and implemented. Much of the information gathered from faculty through the survey is posted for the public via Feinberg Faculty Profiles, to which NMHC's and NMG's websites link.

  • New Users
    For individuals who have not previously completed the annual disclosure survey
  • Returning Users
    For individuals who have completed last year's annual disclosure survey online
  • Facilitators
    For individuals who are responsible for overseeing other survey users

2015 Survey

This year's disclosure survey period has come to a close. An email notification containing a link to the 2015 survey and login instructions will be sent out in winter 2015. Individuals who hold the following roles (organized by institution) are expected to complete the survey:

Northwestern University Feinberg School of Medicine (Feinberg)

  • Full- and Part-Time Faculty (Regular and Research)
  • Contributed Services, Health System Clinician, and Regular-Lecturer Faculty
  • Other Faculty (Adjunct, Emeritus, or Co-Terminus, if also a NMG Physician Consultant or NMH Medical Staff)

Northwestern Memorial HealthCare (NMHC)

  • Board and Committee Members
  • Corporate Officers, Senior Leadership, and Management
  • Medical and Doctoral Level Staff
  • Medical Device and P&T Committee Members and Invitees
  • Mid-Level Providers
  • Other Decision Makers (e.g., Purchasing/Materials Management Staff, Pharmacy Staff, Social Worker/Case Management Staff)

Northwestern Medical Group (NMG)

  • Board Members
  • Senior Leadership and Management
  • Physician Members and Consultants
  • Full-Time and Part-Time Physicians/Employees
  • Mid-Level Providers

Children’s Hospital of Chicago Medical Center (Lurie Children's)

  • Board and Committee Members
  • Corporate Officers, Senior Leadership, Senior Management (Administrators/Directors), and Other Management
