The fundamental mission of the Northwestern University Feinberg School of Medicine (Feinberg), Northwestern Memorial HealthCare (NMHC), Northwestern Medical Faculty Foundation (NMFF), The Children's Memorial Medical Center (Children's Memorial), and the Rehabilitation Institute of Chicago (RIC) is to advance the health of the public. This mission is critically dependent upon the public's trust in the integrity of the research, education, and patient care performed at our institutions. To that end, our academic medical community's relationships with industry or other external entities, which often result in breakthroughs in medical research and promising new therapies, should be encouraged, but also keenly monitored. These relationships must not be or appear to be influenced by factors other than the pursuit of knowledge and the best interests of the patient. With this in mind, Feinberg, NMHC, NMFF, Children's Memorial, and RIC have adopted policies that require their physicians, faculty, management, and certain staff to disclose industry and certain other external relationships through an annual survey. Each person's responses are reviewed, and, if necessary, individual conflict management plans are developed and implemented. Additionally, most of the information gathered from faculty through the survey is posted for our patients and the public on the Feinberg website, to which NMHC's and NMFF's websites link.

This year's disclosure survey is currently underway. The email notification containing a link to the survey website and login instructions was sent out on February 1, 2012. The deadline for completing the survey is February 29, 2012. For your reference, the following types of individuals are expected to complete a survey:

Northwestern University Feinberg School of Medicine (Feinberg)

  • Full- and Part-Time Faculty (Regular and Research)
  • Unpaid Faculty (Contributed Services, Health System Clinician, and Regular-Lecturer) Faculty
  • Other Faculty (Adjunct, Emeritus, or Co-Terminus, if also a NMFF Physician Consultant or NMH Medical Staff)

Northwestern Memorial HealthCare (NMHC)

  • Board and Committee Members
  • Corporate Officers, Senior Leadership, and Management
  • Medical and Doctoral Level Staff
  • Mid-Level Providers
  • Other Decision Makers (e.g., purchasing staff)

Northwestern Medical Faculty Foundation (NMFF)

  • Board Members
  • Senior Leadership and Management
  • Physicians

The Children's Memorial Medical Center (Children's Memorial)

  • Board and Committee Members
  • Corporate Officers, Senior Leadership, and Management
  • Medical and Doctoral Level Staff
  • Mid-Level Providers
  • Clinical Research Professionals

The Rehabilitation Institute of Chicago (RlC)

  • Physical Medicine & Rehabilitation Department

If you have one or multiple roles identified above, please find and select the statement below that best applies to you. You will be redirected to a page with specific information about the survey process and what will be expected of you beginning on February 1, 2012.
