Annual Disclosure of External Professional Relationships Survey

Northwestern University Feinberg School of Medicine (Feinberg), Northwestern Memorial HealthCare (NMHC), Children’s Hospital of Chicago Medical Center (Lurie Children's), and the Shirley Ryan AbilityLab (SRALAB) have adopted policies that require physicians, faculty, management, and certain staff to disclose industry and certain other external relationships through an annual survey. Each person's responses are reviewed, and, if necessary, individual conflict management plans are developed and implemented. Much of the information gathered from faculty through the survey is posted for the public via Feinberg Faculty Profiles, to which NMHC's website links.

2024 Survey

This year's disclosure survey will launch on February 1, 2024, covering external professional relationships and/or activities for the calendar year 2023. The deadline to submit your annual disclosure survey will be February 29, 2024. An email notification containing a link to the 2024 survey and login instructions will be sent out in 2024. Individuals who hold the following roles (organized by the institution) are expected to complete the survey:

Northwestern University Feinberg School of Medicine (Feinberg)

  • Full- and Part-Time Faculty (Regular and Research)
  • Contributed Services, Health System Clinician, and Regular-Lecturer Faculty
  • Other Faculty (Adjunct, Emeritus, or Co-Terminus, if also an NMG Physician Consultant or NMH Medical Staff)

Northwestern Memorial HealthCare (NMHC*)

  • Board and Committee Members
  • Corporate Officers and Senior Leadership
  • Management
  • Employed Physicians and Physician Consultants
  • Medical and Doctoral Staff
  • Advanced Practice Providers
  • Medical Device and P&T Committee Members and Invitees
  • Other Decision Makers (e.g., Purchasing/Materials Management Staff, Pharmacy Staff, Social Work Staff)

*In 2024, surveyed roles may vary by subsidiaries.
