Fellowship Application and Selection Process
To be considered for a CCM, PM*, or CT fellowship position, you must be eligible for certification or fully certified by the American Board of Anesthesiology or the equivalent before fellowship training begins. In addition, you need to obtain an Illinois Permanent License and have taken USMLE Step 3 prior to training.
*Candidates applying to the Multidisciplinary Pain Medicine Fellowship can be a physician eligible for PM & R certification or fully certified.
If you would like to be considered for any fellowship position, please send the following application materials to the appropriate coordinator listed below:
Fellowship Online Application - Complete the on-line application and e-mail saved copy to the appropriate coordinator
A current curriculum vitae
Two letters of recommendation from anesthesiologists with whom you have worked during your residency training
A letter of recommendation from your anesthesiology training program director or chair
A copy of your medical school transcripts
The results of your most two recent ABA/ASA in-training examinations and USMLE Steps I-III exams.
Copies of your CA-2 and 3 Summary Performance Evaluations
A summary of your clinical experience and a brief statement explaining your interest in pursuing a fellowship position.

After the fellowship program director has received and reviewed all of the above-mentioned materials, he or she will notify the coordinator the status of your application and whether or not you will be invited to an interview.
Illinois Department of Financial & Professional Regulations (IDFPR)
Applicants must obtain a Temporary or Permanent Illinois Medical License before starting their fellowship training. Licensing information and application can be found at www.idfpr.com. In order to apply for a permanent license, the applicant must already have taken the USMLE Step III. Please be aware that it may take up to 60 days for IDFPR to process and grant your Illinois license.
International Medical Graduates (IMGs)
Applicants who have not graduated from a residency program in the United States are only eligible to apply for an non-accredited fellowship position in Ambulatory, Neurosurgical, Obstetric, or Regional Anesthesiology. All international medical graduates must be certified by ECFMG before starting their fellowship training. Please refer to www.ecfmg.org for information about eligibility for the examination, fees, application, scheduling, and preparation. Fellowship applicants must submit documentation with an English translation. Photocopies of all examination results, letter/score results, and visa/citizenship papers must bear official seals and include dates and certificate numbers. If the applicant possesses a current visa, the status must include entry and expiration dates. IMGs must possess an ECFMG certificate before applying for a permanent medical license. McGaw Medical Center of Northwestern University sponsors H1-B and J-1 Visas.
