Physicianship forms

Students who demonstrate unprofessional behavior may receive a physicianship form from a course or clerkship director and in some instances a dean. These forms will be sent to the Associate Dean for Medical Education and Competency Achievement. Students who receive a form will need to meet with the Associate Dean for Medical Education and Competency Achievement. If a student receives three forms for unprofessional behavior while a medical student, he or she will need to appear before the Student Promotions Committee. In circumstances when extremely inappropriate behavior is exhibited, a student may be required to come before the Student Promotions Commiteee on the grounds of a single physicianship form.
